Pharmacodynamics and pharmacokinetics

Pharmacodynamics

Angiotensin converting enzyme inhibitors. Chemically fosinopril sodium   - Fozinoprilata ether sodium salt.
Fozinoprilat   It is a selective blocker of competitive angiotensin-converting enzyme . After inhibition of ACE, it prevents the transformation angiotensin   first type angiotensin   the second type having vasopressor   action. This leads to a decrease in the content of angiotensin   a second type in the blood - this leads to a decrease in its vasoconstrictor activity and secretion loosening aldosterone .

Fozinoprilat   hampers the exchange of bradykinin   and therefore the antihypertensive effect is enhanced. Reduced pressure is not accompanied by a decrease or an increase in circulating blood volume, renal and cerebral blood flow, blood supply to organs. After an internal reception antihypertensive   effect occurs within one hour, becomes maximum after 2-5 hours and lasts about one day. A few weeks necessary to achieve the maximum therapeutic effect.

Antihypertensive   effects of thiazide diuretics and fozinoprila   They complement each other.

The drug reduces the sensitivity to physical activity, decrease the severity of heart failure.

Pharmacokinetics

After oral administration the absorption from the intestine reaches 30-40% and is independent of the meal. The maximum plasma concentration occurs within three hours. The interaction with blood proteins is 95%.

Enzymatic hydrolysis of the drug in the liver and intestinal mucosa. Fozinoprilat   It appears equally through the digestive tract and the kidneys. The half-life is close to 11 hours.

Instructions for use Monopril (method and dosage)

Instructions for use Monopril recommends taking the drug inside. The dosage is adjusted individually.

Hypertension . Starting dose - 10 mg once a day. Pick up the dose should be watching the dynamic pressure reduction. The standard dose is in the range 11-40 mg once a day. If sufficient hypotensive effect is not possible to further designate diuretics .

Heart failure . The initial dose of - 5 mg up to two times per day. Over time, the dose is permitted to increase up to 40 mg per day.

Overdose

Symptoms: severe reduction in pressure, bradycardia, shock , Disruption of water and mineral balance, acute course renal failure, stupor.

Treatment: stop taking Monopril, gastric lavage, the use of sorbents, vasopressor agents , Intravenous administration of saline, then symptomatic and supportive therapy. Hemodialysis   ineffective.

Cooperation

Simultaneous use with antacids   reduces the absorption of fozinoprila , So they should be consumed with an interval of 2 hours.

In a joint application with lithium salts   increased risk of toxicity lithium .

Nonsteroidal anti-inflammatory agents   inhibit antihypertensive   Monoprix and the action of these drugs.

When using the drug with diuretics   may develop an excessive pressure drop.

Formulations containing potassium   and potassium-sparing diuretics   increase the likelihood of hyperkalemia .

Fosinopril   activates hypoglycemic   effect insulin   and derivatives sulfonylureas ; increases the risk of leukopenia   when used with cytostatic agents, immunosuppressants, allopurinol, procainamide.

Estrogens   inhibit antihypertensive   Monoprix effect.
